NorthKey case management services provide assistance to individuals (and their familes/guardians) with a diagnosed IDD. Case management services help individuals (and families/guardians) to access services, monitor services being received, coordinate existing or planned services and, in general, provide important advocacy for the IDD client.
Case management services are flexible and unique to each individual’s specific needs. The types of support provided can vary.
